2 | | Be it enacted by the People of the State of Illinois,
|
3 | | represented in the General Assembly:
|
4 | | Section 5. The Compassionate Use of Medical Cannabis Pilot |
5 | | Program Act is amended by changing Section 55 as follows: |
6 | | (410 ILCS 130/55) |
7 | | (Section scheduled to be repealed on January 1, 2018)

8 | | Sec. 55. Registration of qualifying patients and |
9 | | designated caregivers.
|
10 | | (a) The The Department of Public Health shall issue |
11 | | registry identification cards to qualifying patients and |
12 | | designated caregivers who submit a completed application, and |
13 | | at minimum, the following, in accordance with Department of |
14 | | Public Health rules:
|
15 | | (1) A written certification, on a form developed by the |
16 | | Department of Public Health and issued by a physician, |
17 | | within 90 days immediately preceding the date of an |
18 | | application;
|
19 | | (2) upon the execution of applicable privacy waivers, |
20 | | the patient's medical documentation related to his or her |
21 | | debilitating condition and any other information that may |
22 | | be reasonably required by the Department of Public Health |
23 | | to confirm that the physician and patient have a bona fide |
