AMENDMENT TO HOUSE BILL 4332

2    AMENDMENT NO. ______. Amend House Bill 4332 by replacing
3everything after the enacting clause with the following:
 
4    "Section 5. The Health Care Worker Background Check Act is
5amended by changing Section 26 as follows:
 
6    (225 ILCS 46/26)
7    Sec. 26. Health Care Worker Registry. The Department shall
8establish and maintain the Health Care Worker Registry, a
9registry of all individuals who (i) have satisfactorily
10completed the training required by Section 3-206 of the
11Nursing Home Care Act, Section 3-206 of the MC/DD Act, or
12Section 3-206 of the ID/DD Community Care Act, (ii) have begun
13a current course of training as set forth in Section 3-206 of
14the Nursing Home Care Act, Section 3-206 of the MC/DD Act, or
15Section 3-206 of the ID/DD Community Care Act, or (iii) are
16otherwise acting as a nursing assistant, habilitation aide,

1home health aide, or child care aide. The Health Care Worker
2Registry shall include the individual's name, current address,
3and Social Security number or federal taxpayer identification
4number, the date and location of the training course completed
5by the individual, whether the individual has any of the
6disqualifying convictions listed in Section 25 of the Health
7Care Worker Background Check Act from the date of the training
8course completed by the individual, and the date of the
9individual's last criminal records check. Any individual
10placed on the Health Care Worker Registry is required to
11inform the Department of any change of address within 30 days
12after the effective date of the change of address.
13    The Department shall include in the Health Care Worker
14Registry established under this Section the information
15contained in the registries established under Section 3-206.01
16of the Nursing Home Care Act, Section 3-206.01 of the MC/DD
17Act, and Section 3-206.01 of the ID/DD Community Care Act.
