| 2 | | AMENDMENT NO. ______. Amend Senate Bill 84 by replacing | 3 | | everything after the enacting clause with the following:
| 4 | | "Section 5. "An Act in relation to governmental matters", | 5 | | approved June 23, 1995, Public Act 89-29, is amended by | 6 | | changing Section 1-95 as follows: | 7 | | (P.A. 89-29, Sec. 1-95)
| 8 | | Sec. 1-95. The Illinois Department of Mental Health and | 9 | | Developmental
Disabilities, on behalf of the State of | 10 | | Illinois, is authorized and directed to
transfer and convey at | 11 | | a price of $1.00, by the
execution of a Quit Claim Deed, to the | 12 | | Kreider Services, Inc. an Illinois Not
for Profit Corporation, | 13 | | the following described real property, all in Lee
County, | 14 | | Illinois:
| 15 | | Part of the East Half of the East Half of the Southeast | 16 | | Quarter of Section
29, Township 22 North, Range 9 East of |

lands herein described.
| 3 | | The instrument or instruments of conveyance shall be filed | 4 | | for record in the
Office of the Recorder of Deeds of Lee County | 5 | | by Kreider within 15 days after
the delivery of the instrument | 6 | | or instruments or conveyance.
| 7 | | Kreider shall agree that the land conveyed, as described | 8 | | in
this Section, shall be used for the purpose of offering | 9 | | services to individuals
with developmental disabilities or | 10 | | behavioral health needs, or both, Developmental Disabilities | 11 | | pursuant to Kreider's Articles of Incorporation
including, but | 12 | | not limited to, Sheltered Workshop, Work Activity, Adult Day
| 13 | | Training, Residential, and Services for individuals from birth | 14 | | to three years
of
age and if not used for these purposes or if | 15 | | Kreider's use for these purposes
is
discontinued, or if | 16 | | Kreider shall convey or attempt to convey all or any
portion of | 17 | | the land to another party , the land shall revert to ownership | 18 | | by the
State of Illinois. Kreider may convey by quitclaim deed | 19 | | all or any
portion of the land to another party, subject to the | 20 | | condition that if the land is no longer used for the purpose of | 21 | | offering services to individuals
with developmental | 22 | | disabilities or behavioral health needs, or both, the land | 23 | | shall revert to ownership by the
State of Illinois. Any deed or | 24 | | deeds executed under this Article shall
include appropriate | 25 | | provisions to effectuate the purpose of this Section.
