AMENDMENT TO HOUSE BILL 4112

2    AMENDMENT NO. ______. Amend House Bill 4112, AS AMENDED,
3with reference to page and line numbers of House Amendment No.
41, on page 3, line 22 by replacing "January 1, 2026" with "July
51, 2026"; and
 
6on page 3, line 23, by replacing "January" with "July"; and
 
7by deleting line 7 on page 9 through line 7 on page 10; and
 
8on page 11, immediately below line 11, by inserting the
9following:
10    "(c) No group policy of accident and health insurance that
11provides pregnancy related benefits may be issued, amended,
12delivered, or renewed in this State on or after January 1, 2026
13unless the policy contains coverage for the diagnosis and
14treatment of infertility, including, but not limited to, in
15vitro fertilization, uterine embryo lavage, embryo transfer,

1artificial insemination, gamete intrafallopian tube transfer,
2zygote intrafallopian tube transfer, and low tubal ovum
3transfer and procedures necessary to screen or diagnose a
4fertilized egg before implantation, including, but not limited
5to, preimplantation genetic diagnosis, preimplantation genetic
6screening, and prenatal genetic diagnosis. Coverage under this
7subsection for the diagnosis and treatment of infertility
8shall be required only if the procedures:
9        (1) are considered medically appropriate by the
10    patient's medical provider based on clinical guidelines or
11    standards developed by the American Society for
12    Reproductive Medicine, the American College of
13    Obstetricians and Gynecologists, or the Society for
14    Assisted Reproductive Technology; and
15        (2) are performed at medical facilities or clinics
16    that conform to the American College of Obstetricians and
17    Gynecologists guidelines for in vitro fertilization or the
18    American Society for Reproductive Medicine minimum
19    standards for practices offering assisted reproductive
20    technologies."; and
 
21on page 11, line 12, by replacing "(c)" with "(d) (c)"; and
 
22on page 12, line 1, by replacing "(d)" with "(e) (d)"; and
 
23on page 12, line 15, by replacing "(e)" with "(f)"; and
