HOUSE RESOLUTION

WHEREAS, Community Health Partnership of Illinois (CHP) is a Federally Qualified Health Center (FQHC) whose mission is to build and deliver quality, culturally responsive health care to the patients, caregivers, and communities it serves; and

WHEREAS, CHP empowers individuals to attain their best health and well-being and has been serving the uninsured, the underserved, and migrant and seasonal farmworkers and their families in rural Illinois for more than 50 years; and

WHEREAS, CHP's network is comprised of six health and dental clinics and multiple outreach sites located across northern and central Illinois, and its health programs permit employers with access to a qualified and thriving workforce, ensuring Illinois has a strong and healthy economy; and

WHEREAS, CHP also provides students and their families with much needed preventative health services at its physical locations and from its mobile medical units with integrated programs of care, linking and leading to the attainment of better health outcomes; and

WHEREAS, CHP has been recognized by leaders of business, industry, and rural communities for the services it provides while simultaneously bridging and closing the gap in health care; and

WHEREAS, The CHP Kankakee Health Center will be celebrating National Health Center Week and hosting an open house on August 13, 2024; therefore, be it

RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE ONE HUNDRED THIRD GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that we declare August 13, 2024 as Community Health Partnership of Illinois Day in the State of Illinois; and be it further

RESOLVED, That a suitable copy of this resolution be presented to Community Health Partnership of Illinois as a symbol of our respect and esteem.
