Updating the database of the Illinois Compiled Statutes (ILCS) is an ongoing process.
Recent laws may not yet be included in the ILCS database, but they are found on this site as
Public
Acts soon after they become law. For information concerning the relationship between statutes and Public Acts, refer to the
Guide.
Because the statute database is maintained primarily for legislative drafting purposes,
statutory changes are sometimes included in the statute database before they take effect.
If the source note at the end of a Section of the statutes includes a Public Act that has
not yet taken effect, the version of the law that is currently in effect may have already
been removed from the database and you should refer to that Public Act to see the changes
made to the current law.

Sec. 55. Change of address or name; lost, destroyed, or stolen licenses. (a) A licensee shall notify the Illinois State Police within 30 days of moving or changing residence or any change of name. The licensee shall submit the requisite fee and the Illinois State Police may require a notarized statement that the licensee has
changed his or her residence or his or her name, including the prior and current address or name and the date the applicant moved or changed his or her name. (b) A licensee shall notify the Illinois State Police within 10 days of discovering that a license has been lost, destroyed, or stolen. A lost, destroyed, or stolen license is invalid. To request a replacement license, the licensee shall submit: (1) a written or electronic acknowledgment that the |
| licensee no longer possesses the license, and that it was lost, destroyed, or stolen;

(2) if applicable, a copy of a police report stating
|
| that the license was stolen; and
|
|
(3) the requisite fee.
(c) A violation of this Section is a petty offense with a fine of $150 which shall be deposited into the Mental Health Reporting Fund.
