changes in Section requiring the Department of Corrections to
maintain records of committed persons.

1 AN ACT to amend the Unified Code of Corrections by
2 changing Section 3-5-2.
3 Be it enacted by the People of the State of Illinois,
4 represented in the General Assembly:
5 Section 5. The Unified Code of Corrections is amended by
6 changing Section 3-5-2 as follows:
7 (730 ILCS 5/3-5-2) (from Ch. 38, par. 1003-5-2)
8 Sec. 3-5-2. Institutional Record.
9 The Department shall maintain records of the examination,
10 assignment, transfer, discipline of committed persons and
11 what grievances, if any, are made in each of its
12 institutions, facilities and programs. The record shall
13 contain the name of the persons involved, the time, date,
14 place and purpose of the procedure, the decision and basis
15 for the decision therefor, and any review of the decision
16 made.
