Provides that for the purpose of ensuring that each child receives age-appropriate immunizations, the Department of Public Health and any physician, licensed institutional health care provider, and local health department may share immunization and child locator information. The Communicable Disease Prevention Act is 6 amended by adding Section 4 as follows: 7 (410 ILCS 315/4 new) 8 Sec. 4. Sharing information. For the purpose of 9 ensuring that each child receives age-appropriate 10 immunizations, the Department of Public Health and any 11 physician, licensed institutional health care provider, and 12 county, multiple county, or other local health department may 13 share immunization and child locator information. The 14 information may be shared only with the written consent of a 15 child's parent or legal guardian. The information may 16 include, but need not be limited to, the following: 17 (1) The month, day, and year that each immunization 18 was administered to the child. 19 (2) The child's name, address, telephone number, 20 birth date, and social security number. 21 (3) The name or names of the child's parent or 22 parents or other legal custodian. 23 The information described in items (1), (2), and (3) is 24 confidential and may be shared only for the purposes set 25 forth in this Section.
