 1                    AMENDMENT TO SENATE BILL 1372
 2        AMENDMENT NO.     .  Amend Senate Bill 1372 as follows:
 3    on page 1, by replacing lines 1 and 2 with the following:
 4        "AN ACT concerning human services  and  health  services,
 5    amending named Acts."; and
 6    between lines 4 and 5, by inserting the following:
 7        "Section   2.   The   Mental   Health  and  Developmental
 8    Disabilities Administrative Act is amended by adding  Section
 9    31b as follows:
10        (20 ILCS 1705/31b new)
11        Sec.   31b.  Individuals   waiting   for  community-based
12    services.   The  Department  shall   collect   and   maintain
13    information  on  the number of individuals with developmental
14    disabilities who have requested (either themselves or through
15    a parent or guardian) and  are  waiting  for  community-based
16    services  that  are funded through grants from the Department
17    to, and contracts  with  the  Department  and,  local  school
18    districts, individual service coordination organizations, and
19    community   based   organizations.    The  information  shall
20    include, but is not limited to, the following:

 1             (1)  The number of  those  individuals  waiting  for
 2        community-based  residential  services  such as Community
 3        Integrated  Living  Arrangements,  supported  apartments,
 4        supported   living,   and    other    community    living
 5        arrangements.
 6             (2)  The  number  of  those  individuals waiting for
 7        services  from  intermediate  care  facilities  for   the
 8        developmentally disabled.
 9             (3)  The  number  of  those  individuals waiting for
10        services from day and vocational  programs  such  as  day
11        activity   programs,   adult   day   care,  developmental
12        training, sheltered workshops, supported employment,  job
13        placement, and other day programs.
14             (4)  The  number  of  those  individuals waiting for
15        family   support   services,   respite   care,   personal
16        assistance  services,  case  management  services,  early
17        intervention services, and other services not included in
18        paragraph (3).
19             (5)  The ages of the those individuals  waiting  for
20        services under paragraphs (1), (2), (3), and (4).
21             (6)  The  ages  of  the parents of those individuals
22        waiting for services under paragraphs (1), (2), (3),  and
23        (4).
24             (7)  The  Representative  Districts  in  which those
25        individuals waiting for services live.
26    The information shall be updated annually.   The  information
27    collected  shall  be limited to information about individuals
28    who are eligible for services at the time of request  or  who
29    will  be  eligible to receive services within 12 months after
30    the date services are requested.  The  Department  shall,  on
31    request,  make  the  information  available to other agencies
32    serving persons with developmental disabilities.".
