1 SENATE RESOLUTION
2 WHEREAS, The members of the Senate were saddened to learn
3 of the death of Rose Ann Van Slyke of Plainfield on Sunday,
4 November 15, 1998; and
5 WHEREAS, She was born on November 1, 1945 in Tecumseh,
6 Nebraska, the daughter of Loretta Vivian Puchalla and the
7 late Elmer A. Puchalla; and
8 WHEREAS, She had been a registered nurse in Naperville
9 area for the past 28 years; she was an active member of St.
10 Mary Immaculate Catholic Church in Plainfield; and
11 WHEREAS, Her passing will be deeply felt by her family
12 and friends, especially her husband, Dallas; her daughters,
13 Tracy Noble, Jennifer Van Slyke, and Shelly Van Slyke; her
14 son-in-law, James Noble; her mother, Loretta Vivian Puchalla;
15 her sister, Ruth Acker; and her brothers, John and Don
16 Puchalla; therefore, be it
17 RESOLVED, BY THE SENATE OF THE NINETIETH GENERAL ASSEMBLY
18 OF THE STATE OF ILLINOIS, that we note with sorrow and regret
19 the death of Rose Ann Van Slyke and extend our most sincere
20 condolences to her family and friends; and be it further
21 RESOLVED, That a suitable copy of this resolution be
22 presented to her husband, Dallas Van Slyke.
