1 AN ACT to amend the Illinois Food, Drug and Cosmetic Act.
2 Be it enacted by the People of the State of Illinois,
3 represented in the General Assembly:
4 Section 5. The Illinois Food, Drug and Cosmetic Act is
5 amended by changing Section 2.36 as follows:
6 (410 ILCS 620/2.36) (from Ch. 56 1/2, par. 502.36)
7 Sec. 2.36. "Prescription" means and includes any order
8 for drugs or medical devices, whether written, facsimile, or
9 verbal, by a physician licensed to practice medicine in all
10 its branches, a dentist, a veterinarian, or a podiatrist, and
11 that contains containing the following: (1) name of the
12 patient; (2) date when prescription was given; (3) name and
13 strength of drug or description of the medical device
14 prescribed; (4) quantity, (5) directions for use, (6)
15 prescriber's name, address and signature, and (7) DEA number
16 where required, for controlled substances.
