Provides for the right of each patient, upon the written request of the patient or the patient's guardian or legally authorized representative, to be furnished a copy of the record of the patient's health history and treatment rendered to the patient by a physician or other health care provider. Limits the handling fee that may be charged for providing records to $10 plus a fee of not more than $.35 per page for copies of documents made on a standard photocopy machine.
