92_HB5939enr HB5939 Enrolled LRB9211085NTpkB 1 AN ACT concerning schools. 2 Be it enacted by the People of the State of Illinois, 3 represented in the General Assembly: 4 Section 5. The School Code is amended by adding Sections 5 10-20.35 and 34-18.23 as follows: 6 (105 ILCS 5/10-20.35 new) 7 Sec. 10-20.35. Medical information form for bus drivers 8 and emergency medical technicians. School districts are 9 encouraged to create and use an emergency medical information 10 form for bus drivers and emergency medical technicians for 11 those students with special needs or medical conditions. The 12 form may include without limitation information to be 13 provided by the student's parent or legal guardian concerning 14 the student's relevant medical conditions, medications that 15 the student is taking, the student's communication skills, 16 and how a bus driver or an emergency medical technician is to 17 respond to certain behaviors of the student. If the form is 18 used, the school district is encouraged to notify parents and 19 legal guardians of the availability of the form. The parent 20 or legal guardian of the student may fill out the form and 21 submit it to the school that the student is attending. The 22 school district is encouraged to keep one copy of the form on 23 file at the school and another copy on the student's school 24 bus in a secure location. 25 (105 ILCS 5/34-18.23 new) 26 Sec. 34-18.23.
