92_SB0935ham001 LRB9207852JSpcam 1 AMENDMENT TO SENATE BILL 935 2 AMENDMENT NO. . Amend Senate Bill 935 on page 1 by 3 inserting immediately below line 3 the following: 4 "Section 3. The State Employees Group Insurance Act of 5 1971 is amended by changing Section 6.11 as follows: 6 (5 ILCS 375/6.11) 7 Sec. 6.11. Required health benefits; Illinois Insurance 8 Code requirements. The program of health benefits shall 9 provide the post-mastectomy care benefits required to be 10 covered by a policy of accident and health insurance under 11 Section 356t of the Illinois Insurance Code. The program of 12 health benefits shall provide the coverage required under 13 Sections 356u, 356w, and 356x of the Illinois Insurance Code. 14 The program of health benefits must comply with Section 15 155.37 of the Illinois Insurance Code. 16 (Source: P.A. 90-7, eff. 6-10-97; 90-655, eff. 7-30-98; 17 90-741, eff. 1-1-99.)"; and 18 on page 1, line 5, by changing "Section 155.37" to "Sections 19 155.37, 370t, and 511.114"; and 20 on page 1 by inserting immediately below line 13 the 21 following: -2- LRB9207852JSpcam 1 "(215 ILCS 5/370t new) 2 Sec. 370t. Drug formulary; notice. All administrators 3 must comply with Section 155.37 of this Code. 4 (215 ILCS 5/511.114 new) 5 Sec. 511.114. Drug formulary; notice. All 6 administrators must comply with Section 155.37 of this Code. 7 Section 10. The Comprehensive Health Insurance Plan Act 8 is amended by adding Section 8.7 as follows: 9 (215 ILCS 105/8.7 new) 10 Sec. 8.7. Drug formulary; notice. The plan must comply 11 with Section 155.37 of the Illinois Insurance Code. 12 Section 15. The Health Maintenance Organization Act is 13 amended by changing Section 4-6.5 as follows: 14 (215 ILCS 125/4-6.5) 15 Sec. 4-6.5. Required health benefits; Illinois Insurance 16 Code requirements. A health maintenance organization is 17 subject to the provisions of Sections 155.37, 356t, and 356u 18 of the Illinois Insurance Code. 19 (Source: P.A. 90-7, eff. 6-10-97.) 20 Section 20. The Limited Health Service Organization Act 21 is amended by changing Section 4003 as follows: 22 (215 ILCS 130/4003) (from Ch. 73, par. 1504-3) 23 Sec. 4003. Illinois Insurance Code provisions. Limited 24 health service organizations shall be subject to the 25 provisions of Sections 133, 134, 137, 140, 141.1, 141.2, 26 141.3, 143, 143c, 147, 148, 149, 151, 152, 153, 154, 154.5, 27 154.6, 154.7, 154.8, 155.04, 155.37, 355.2, 356v, 368a, 401, -3- LRB9207852JSpcam 1 401.1, 402, 403, 403A, 408, 408.2, 409, 412, 444, and 444.1 2 and Articles IIA, VIII 1/2, XII, XII 1/2, XIII, XIII 1/2, 3 XXV, and XXVI of the Illinois Insurance Code. For purposes 4 of the Illinois Insurance Code, except for Sections 444 and 5 444.1 and Articles XIII and XIII 1/2, limited health service 6 organizations in the following categories are deemed to be 7 domestic companies: 8 (1) a corporation under the laws of this State; or 9 (2) a corporation organized under the laws of 10 another state, 30% of more of the enrollees of which are 11 residents of this State, except a corporation subject to 12 substantially the same requirements in its state of 13 organization as is a domestic company under Article VIII 14 1/2 of the Illinois Insurance Code. 15 (Source: P.A. 90-25, eff. 1-1-98; 90-583, eff. 5-29-98; 16 90-655, eff. 7-30-98; 91-549, eff. 8-14-99; 91-605, eff. 17 12-14-99; 91-788, eff. 6-9-00.) 18 Section 25. The Voluntary Health Services Plans Act is 19 amended by changing Section 10 as follows: 20 (215 ILCS 165/10) (from Ch. 32, par. 604) 21 Sec. 10.
