5 on page 10, by replacing lines 6 through 9 with the
6 following:

8 and
9 between lines 14 and 15, by inserting the following:
10 "(3) For requesting a copy of a final decree of
11 adoption, relief from a final judgment or order,
12 appointment of a confidential intermediary, or requesting
13 non-identifying information.........................$40".
