1 AMENDMENT TO SENATE BILL 857
2 AMENDMENT NO. . Amend Senate Bill 857 on page 27,
3 immediately below line 13, by inserting the following:
4 "Section 17. The Illinois Dental Practice Act is amended
5 by changing Section 21 as follows:
6 (225 ILCS 25/21) (from Ch. 111, par. 2321)
7 Sec. 21. Fees. The fees for the administration and
8 enforcement of this Act, including but not limited to
9 original licensure, renewal, and restoration fees, shall be
10 set by the Department by rule. However, the fee for
11 application for renewal of a license as a dentist or
12 specialist is $75 per year and the fee for application for
13 renewal of a license as a dental hygienist is $37.50 per
14 year. The fees shall be nonrefundable. The Department is
15 authorized to impose the following nonrefundable fees:
16 (a) License as dentist or specialist.
17 (1) Application for temporary training or
18 restricted faculty license: $25.
19 (2) Application for initial license pursuant to
20 examination: $25.
21 (3) Applicants for any examination shall be
22 required to pay, either to the Department or to the

1 designated testing service, a fee covering the cost of
2 initial screening to determine eligibility and providing
3 the examination.
4 (4) Application for licensure from a person
5 licensed as a dentist under the laws of another state or
6 territory of the United States: $50.
7 (5) Application for renewal of a license: $50 per
8 year.
9 (6) Application for restoration of a license: $10
10 plus payment of all lapsed renewal fees.
11 (7) Application for inactive status: $10.
12 (8) Application for restoration from inactive
13 status: the current renewal fee.
14 (9) Application for restoration after failure to
15 notify Department of change of address: $15, plus payment
16 of all lapsed renewal fees.
17 (10) Application for an initial permit to
18 administer general anesthesia or parenteral conscious
19 sedation: $25.
20 (11) Application for renewal of a permit to
21 administer general anesthesia or parenteral conscious
22 sedation: $25 per year.
23 (b) License as dental hygienist.
24 (1) Application for initial license pursuant to
25 examination: $25.
26 (2) Applicants for any examination shall be
27 required to pay, either to the Department or to the
28 designated testing service, a fee covering the cost of
29 initial screening to determine eligibility and providing
30 the examination.
31 (3) Application for license from a person licensed
32 as a dental hygienist under the laws of another state or
33 territory of the United States: $50.
34 (4) Application for renewal of a license: $15 per

1 year.
2 (5) Application for restoration of a license: $10
3 plus payment of all lapsed renewal fees, but not to
4 exceed $85.
5 (6) Application for inactive status: $10.
6 (7) Application for restoration from inactive
7 status: the current renewal fee.
8 (c) General fees.
9 (1) Application for a duplicate license: $20.
10 (2) Application for a certification of a
11 registrant's record for any purpose: $20.
12 (3) Application to have the scoring of an
13 examination administered by the Department reviewed and
14 verified: $20, plus any fees charged by the applicable
15 testing service.
16 (4) The fee for a wall certificate showing a
17 license shall be the actual cost of producing such
18 certificate.
19 (5) The fee for a roster of persons licensed as
20 dentists or dental specialists or dental hygienists in
21 this State shall be the actual cost of producing such a
22 roster.
23 (6) The fee for dental licensing, disciplinary or
24 investigative records pursuant to a subpoena is $1 per
25 page.
