 1        AN ACT concerning vehicles.

 2        Be  it  enacted  by  the People of the State of Illinois,
 3    represented in the General Assembly:

 4        Section 5.  The  Illinois  Vehicle  Code  is  amended  by
 5    adding Sections 3-405.5 and 6-106.5 as follows:

 6        (625 ILCS 5/3-405.5 new)
 7        Sec.  3-405.5.  Registration check-off for blindness. The
 8    Secretary of State shall,  in  a  conspicuous  place  on  its
 9    standard   application   for  vehicle  registration  and  its
10    standard application for vehicle registration renewal,  print
11    a  box and a provision indicating that if the person applying
12    for registration or renewal wants to  contribute  $1  to  the
13    Blindness  Prevention  Fund  as authorized by this amendatory
14    Act of the 93rd General Assembly, he or  she  may  do  so  by
15    checking  the  designated  box  and paying that amount to the
16    Secretary of State in addition to the registration or renewal
17    fee.
18        Any contribution for the Blindness Prevention  Fund  made
19    under  this  Section  shall  be  deposited into the Blindness
20    Prevention Fund to provide  blindness  prevention  education,
21    screening, and treatment.

22        (625 ILCS 5/6-106.5 new)
23        Sec.  6-106.5.  Registration check-off for blindness. The
24    Secretary of State shall,  in  a  conspicuous  place  on  its
25    standard  application  for  a  license and for an instruction
26    permit, print a box and a provision indicating  that  if  the
27    person  applying for a license or an instruction permit wants
28    to  contribute  $1  to  the  Blindness  Prevention  Fund   as
29    authorized  by  this  amendatory  Act  of  the  93rd  General
30    Assembly,  he or she may do so by checking the designated box

 1    and paying that amount to the Secretary of State in  addition
 2    to the license or instruction permit fee.
 3        Any  contribution  for the Blindness Prevention Fund made
 4    under this Section shall  be  deposited  into  the  Blindness
 5    Prevention  Fund  to  provide blindness prevention education,
 6    screening, and treatment.

 7        Section 10.  The State Finance Act is amended  by  adding
 8    Section 5.596 and 6z-61 as follows:

10        Sec. 5.596.  The Blindness Prevention Fund.

11        (30 ILCS 105/6z-61 new)
12        Sec.   6z-61.  The   Blindness   Prevention   Fund.   The
13    Blindness Prevention Fund is created as a special fund in the
14    State treasury.  Subject  to  appropriation  by  the  General
15    Assembly  and  approval by the Secretary of State, the moneys
16    in the Blindness Prevention Fund shall be paid as  grants  to
17    the Illinois Division of Prevent Blindness America to provide
18    blindness prevention education, screening, and treatment.
