AMENDMENT TO SENATE BILL 2173

2     AMENDMENT NO. ______. Amend Senate Bill 2173, AS AMENDED,
3 with reference to page and line numbers of Senate Amendment No.
4 1, on page 1, after line 22, by inserting the following:
 
5     "(a-5) "Consumer Choice of Benefits HMO plan" means a
6 health maintenance organization subscriber contract that, in
7 whole or in part, does not place limitations of restrictions on
8 deductibles, coinsurance, copayments, or any annual or
9 lifetime maximum benefit amounts, but that provides creditable
10 coverage as defined by Section 20 of the Illinois Health
11 Insurance Portability and Accountability Act."; and
 
12 on page 2, after line 4, by inserting the following:
 
13     "(e) "Health Maintenance Organization" means any
14 organization formed under the laws of this or another state to
15 provide or arrange for one or more health care plans under a
16 system that causes any part of the risk of health care delivery
17 to be borne by the organization or its providers."; and
 
18 on page 4, by replacing lines 3 and 4 with the following:
 
19     "plans authorized; minimum requirement.
20     (a) An insurer may offer one or more Consumer Choice of
21 Benefits Health Insurance plans.

1     (b) A health maintenance organization may offer one or more
2 Consumer Choice Benefits HMO plans, provided that the plan's
3 deductibles and copayments paid for the receipt of basic health
4 care services do not exceed $5,000 per enrollee or $10,000 per
5 family in a calendar or contract year.".
