AMENDMENT TO HOUSE BILL 2453

AMENDMENT NO. ______. Amend House Bill 2453 on page 1, by replacing lines 9 and 10 with the following:

"post-hospital care, whenever a patient who qualifies for the federal Medicare program is hospitalized, the patient shall be"; and

on page 1, line 12, by deleting "skilled"; and

on page 1, by replacing lines 14 and 15 with the following: "hospital must inform the designated case coordination unit or other pre-screening entity of the pending discharge"; and

on page 1, line 16, after "unit's", by inserting "or other pre-screening entity's"; and

on page 1, line 26, by changing "assisted living and shared housing" to "assisted living or shared housing"; and

on page 1, line 30, by deleting "or"; and

on page 1, line 31, after "Act", by inserting "; or (vi) any agency licensed under the Home Health Agency Licensing Act"; and

on page 2, line 1, by deleting "annually"; and

on page 2, lines 3 and 4, by changing "nursing homes" to "nursing facilities"; and

on page 2, line 4, by changing "and hospice facilities" to "hospice facilities, and home health agencies"; and

on page 2, line 10, by changing "24 hour" to "24-hour 24 hour".
