1 AMENDMENT TO HOUSE BILL 61
2 AMENDMENT NO. . Amend House Bill 61, AS AMENDED,
3 with reference to the page and line numbers of House
4 Amendment No. 1, on page 2, in lines 15 and 16, by replacing
5 "Illinois Department of Public Health" with "Bureau of
6 Apprenticeship Training of the United States Department of
7 Labor"; and
8 on page 11, in line 5, after "one", by inserting "year"; and
9 on page 12, in line 11, by deleting "All licenses"; and
10 on page 12, in line 12, before "issued", by inserting the
11 following:
12 "(a) All licenses"; and
13 on page 12, by replacing lines 13 through 15 with the
14 following:
15 "provided by the Department.
16 (b) The following fees are nonrefundable:
17 (1) The fee application for a license or temporary
18 license is $100.
19 (2) The fee for the renewal of a license is $25.
20 (3) The fee for the reinstatement of a license that
21 has been expired for less than 2 years is $50, plus
22 payment of all unpaid renewal fees.

1 (4) The fee for the restoration of a license that
2 has been expired for more than 2 years is $100.
3 (5) The fee for the issuance of a duplicate
4 license, the issuance of a replacement license that has
5 been lost or destroyed, or the issuance of a license with
6 a change of name or address, other than during the
7 renewal period, is $25. No fee is required for name and
8 address changes on Department records when no duplicate
9 license is issued.
10 (6) The fee for the certification of a licensee's
11 record for any purpose is $25.
12 (7) The fee for the rescoring of an examination is
13 the cost to the Department of rescoring the examination,
14 plus any fees charged by the applicable testing service
15 to have the examination rescored.
16 (8) The fee for copies of a license shall be the
17 actual cost of producing the copies.
18 (9) The fee for a roster of persons licensed as
19 electricians is the actual cost of producing the roster.
20 (10) The fee for application for a license by an
21 electrician registered or licensed under the laws of
22 another state is $100."; and
23 on page 15, in line 18, by replacing "shall" with "may"; and
24 on page 18, in line 5, by replacing "shall" with "may".
