Amend Senate Bill 781 on page 4, by 3 replacing lines 20 and 21 with the following: 4 "immunization requirements on religious or medical grounds or 5 on grounds of a conscientiously held belief as provided in 6 subsection (8). This reported"; and 7 on page 4, by replacing line 34 with the following: 8 "on religious grounds or on grounds of a conscientiously held 9 belief shall not be required"; and 10 on page 6, by replacing lines 3 and 4 with the following: 11 "immunizing agents conflicts with his or her religious tenets 12 or practices or conscientiously held beliefs; except that if 13 the objection is made on grounds of a conscientiously held 14 belief, the parent or guardian of the child shall submit to 15 the principal of the school that the child is to attend, not 16 later than 45 days before the beginning of the school term, a 17 signed, notarized statement of objection detailing the nature 18 of that conscientiously held belief; or,".
