 1                    AMENDMENT TO HOUSE BILL 1881
 2        AMENDMENT NO.     .  Amend House Bill 1881,  AS  AMENDED,
 3    in  the  introductory  clause  to  Section  30 of the bill by
 4    changing "356t and 356u" to "356t, 356u, and 356v"; and
 5    in  the  body  of  Section  30  of  the  bill  by   inserting
 6    immediately below the last line of Sec. 356u the following:
 7        "(215 ILCS 5/356v new)
 8        Sec. 356v. Treatment of phenylketonuria.
 9        (a)  A group policy of accident and health insurance that
10    is issued, delivered, amended, or renewed in this State on or
11    after January 1, 1998 shall include coverage for prescription
12    enteral and oral formulas for home use, for which a physician
13    licensed  to practice medicine in all its branches has issued
14    a written order and which are  medically  necessary  for  the
15    treatment and management of phenylketonuria.
16        (b)  This   Section   does   not   apply  to  agreements,
17    contracts, or policies that provide coverage for a  specified
18    disease or other limited benefit coverage."; and
19    in  the  body  of  Section  35 of the bill, in Sec. 4-6.5, by
20    replacing "and 356u" with ", 356u, and 356v"; and
21    by inserting immediately below the last line of Section 35 of

 1    the bill the following:
 2        "Section 37.  The Limited Health Service Organization Act
 3    is amended by adding Section 4002.2 as follows:
 4        (215 ILCS 130/4002.2 new)
 5        Sec. 4002.2.  Treatment of  phenylketonuria.   A  limited
 6    Health Service Organization is subject to Section 356v of the
 7    Illinois Insurance Code."; and
 8    in  the  body  of  Section  40  of  the  bill, in Sec. 10, by
 9    changing "356u" to "356u, 356v".
